Registering a New Student
Parents and guardians will need to bring the following documents and forms to enroll a child at Ho'okena Elementarty.
- Student Health Record, including a TB clearance (please the Student Health Record section below for more details)
- Birth Certificate
- Proof of current address
- Documents from a previous school
- Legal documents
Kindergarten
To enter Kindergarten, children must be 5 years old by July 31. We are now registering children for Kindergarten.

Transfer from another Hawai'i School
If your child will be transferring from another Hawai'i school to Ho'okena Elementary, please notify the current school in advance, as much as possible, that your child will be transferring. The current school will prepare your child's paperwork and forward it to our school after you have registered with us.
While you will not need all of the documents listed for a new student, you will need proof of current address and any custodial legal documents, if appropriate.

Transfer from Out of State
Students transferring from a school in another state will need all the documents listed in the new student section.
Student Health Record
A student’s Health Record must be initiated for entry into school. Parents or guardians are asked to complete the minimum requirements of the form before entry into school.

- Hawaii Department of Health - If you will be completing the tuberculosis clearance with the Hawaii Department of Health, they will complete the Hawaii Department of Health Tuberculosis Control Branch form. For testing locations and hours, please click: Hawaii Department of Health Testing Locations and Hours of Operation.
- Primary Care Physician - If you will be completing the tuberculosis clearance with your primary care physician, your physician will provide a different assessment and clearance form.
